Definitions, sources and limits

Each row is a fictional service-month, payer and location. Each claim is observed 90 days after service. Expected payer cash is the collectible payer portion after applicable adjustments and patient responsibility. Posted rate equals total posted payer payments divided by total expected payer dollars. Rates across locations are weighted by dollars.

Collected and posted, deposited but unposted, payer follow-up, and remittance review are mutually exclusive in this example. Banked cash includes the first two categories. A difference alone cannot establish that a fee schedule changed, a payer underpaid, or money is recoverable. The example uses no real beneficiary or client records.

CMS explains how remittance advice identifies payment amounts, adjustments and financial responsibility. Read the CMS guidance.
